At Bridge To Change, our Positive Behaviour Support (PBS) service is built around improving quality of life — not just reducing behaviours of concern. We take a compassionate, evidence-based approach that focuses on understanding the reasons behind challenging behaviours and working with individuals and their support networks to develop respectful, proactive strategies.

Our PBS Practitioners collaborate closely with participants, families, carers, and service providers to build personalised Behaviour Support Plans that are practical, strengths-based, and aligned with NDIS guidelines. Every plan we create is designed to meet the unique goals, needs, and values of the individual.

What Our Positive Behaviour Support Services Include

🔍 Functional Behaviour Assessment (FBA)

We conduct thorough assessments to identify the underlying causes, patterns, and triggers of behaviours of concern. This helps ensure our recommendations are not just reactive, but deeply informed.

📘 Behaviour Support Plan Development

Our team develops comprehensive Behaviour Support Plans (BSPs) that are practical, trauma-informed, and aligned with the participant’s NDIS goals. Plans may include strategies to prevent, respond to, and replace challenging behaviours with positive alternatives.

⚠️ Restricted Practice Assessment & Reporting

If restrictive practices are in place or under consideration, we ensure they are clearly documented, legally compliant, and reviewed regularly. We also support organisations in reporting to the NDIS Quality and Safeguards Commission.

👥 Collaboration with Families & Support Teams

We work closely with the individual’s family, support workers, allied health professionals, and others involved to ensure a consistent and coordinated approach across all environments.

🛠️ Skill Development & Capacity Building

We support participants in building emotional regulation, communication, daily living, and social interaction skills — creating pathways for independence and confidence.

🎓 Training & Coaching for Support Staff

We provide training and hands-on coaching for staff and carers to help them confidently implement strategies, respond effectively to behaviours, and maintain a positive, supportive environment.

🔄 Ongoing Monitoring & Review

Our PBS practitioners provide regular follow-up, progress reviews, and plan updates to ensure the support remains effective, relevant, and flexible as the participant’s needs evolve.

Our Process

Initial Consultation

Fill in an online Referral Form.

Review and sign your online Service Agreement.

We conduct initial interviews, document and data review, observations and interactions

Behaviour Plan Development

We always start with a Quality of Life assessment. We assess your strengths, risks, and goals, and whether there may be a restrictive practice in place.

If a Restrictive Practice is found, we develop a draft Interim Behaviour Support Plan and circulate it to you and your team for feedback.

Once you are happy with it we submit this to the NDIS Commission, if it includes Restrictive Practices.

Implementation & Continuous Review

We provide training and coaching to you, your key support people and your family.

We listen to your ongoing feedback, review your data, and update the Behaviour Plan.

If you have 35 or more NDIS hours, we provide a Comprehensive Behaviour Support Plan. This includes a Functional Behaviour Assessment and:

  1. Function-based Proactive strategies to improve your quality of life:
    1. Environmental strategies,
    2. Skill development strategies,
    3. Focused strategies.
  2. Reactive strategies to increase safety and rapidly sooth challenging behaviour incidents.
  3. Data collection systems to allow ongoing review (e.g. mobile app, or paper form).

Outcome & Recommendation Reports

At the closure of every service, we provide a letter of recommendation, to help you obtain from the NDIS the right level of ongoing support. The NDIA wants evidence to show how reasonable and necessary your support needs are. We are experts in the collection, analysis and delivery of this evidence.

Your behaviour support and lifestyle goals are clearly defined.

Together we build a plan and build the skills to meet these goals.

Your progress towards these goals is measured and delivered to the NDIA in a compelling report.
